IT is a Cat c12. Cat did a complete rebuild 250k miles ago. I have had the truck for a year and its always had this rattle. I have heard one or 2 other trucks with cat motors on site with the same rattle. My mechanic who owns a large fleet, half of which are cat engine equipped told me it will probably make that noise for the rest of its life its just wear. Anyhow, I have OCD and it bugs the crap out of me.
The ratte sounds like a bad tensioner but its not that. I removed the belt and ran the truck. Only thing spinning was the crank pulley and balancer, Still makes the sound. Seems to be coming from the crank area. It is definitely coming from the front of the motor and not the radiator. Noise is too low on motor to be coming from the idler gear but who knows. ONLY makes the sound on a warmed up engine and the sound goes away if you bump the rpms up. No noise when cold so I find it hard to believe its the balancer.

The problem is it's a c12. Get rid of it. I'm not kidding. They are money pits.

Just a thought, when it was rebuilt, did you replace the crankshaft dampner? Although I've never seen or heard of one making such noise, I guess it would be possible.
One truck I drove had a fresh Cat platinum rebuild just before I got in it. That engine had a whine to it at idle that was very hard on the ears. I never did figure it out. It still runs, and still whines. -
Is the video at low idle? Does it go away at high idle? Could be a loose mount, or something possibly.
N14 engine rattle at low idle also. -
First place I would be going is checking gear lash setup on the front gear train. Do you know for certain it was set to specs during In-frame? Have you removed belts to confirm it's not anything accessory?
