Two Arizona truck wash employees are dead after fainting inside a liquid tank semi-truck Sunday while they were cleaning.
While trying to recover the men, four firefighters suffered heat-related injuries and were transported to hospitals, the Arizona Republic reported Monday.
Avondale police said in a release that the first employee of Danny’s Truck Wash, in Avondale, about 20 miles west of Phoenix, was told to clean the inside of the liquid tank only to collapse and fall unconscious shortly after, according to the outlet.

Another employee went to help his colleague and similarly fell unconscious, the paper reports.
Both men were pronounced dead at the scene and recovered from the tank with the arrival of firefighters, with fire officials deeming the incident a full hazardous materials entry and body recovery, according to the outlet.
Officials said a hazardous spill had not occurred and there was not a risk to public safety, according to the Arizona Republic, but the men’s deaths and the four hospitalizations are being investigated as a hazardous materials incident, KTAR News reports.
Avondale Police did not immediately respond to a request for comment.

I’d say Nitrogen blanket.
I always wrote it on BoL.
They’re supposed to check air and have meter on them, but..
And you can’t help without air, you’ll get disoriented and be stuck in the cloud of no oxygen.

I am sorry for their loss. When working in confined space you have to know what atmosphere you are dealing with. Protocols could not have been followed. If the tank atmosphere was not identified you don't proceed and until it is tested & confirmed. The Lord comfort their families.

All falls back on the employer and the supervisor. Their blood is on their hands. Employees were told to go in and clean. They weren't trained to do so and were unqualified. Lots of safety protocols to follow along with proper ventilation and or breathing apparatus all which obviously weren't supplied. And these workers didn't know the dangers of such task. Such a shame. Lawsuit wont bring them back but the employer is gonna learn the hard way.

I've taken confined space training and have been in confined spaces. Protocols were definitely not followed by the sound of it. Gas test/personal monitor, a rescue plan and a trained supervisor at the entry point have always been required whenever I've had to enter a space.
Biggest thing is if somebody goes down and the area is suspected of having fumes or lack of oxygen, do not enter unless you've got the required safety gear and training. 1 casualty can easily become 2 or more.Roger McG, WildTiger1990, mjd4277 and 4 others Thank this. -

I did some sleuthing and ABC15 had some video that appears to show that the tank wasn’t placarded.
