Preventative Maintenance and Vehicle Maintenance Log

    Hey Guys, Hope you all had a wonderful easter. I am wondering if anyone can show me an example of their Preventative Maintenance and Vehicle Maintenance Log, I am a carrier/owner-op out of Washington state. I just need to make sure im doing stuff correctly before i get audited and fail.

    Unfortunately I'm not home or I would. Did you get a New Entrant Audit Guide from DOT? There are copies of how your Maintenance Log should be set up found within that guide.

    @ Straight Stacks, Yeah they did, i just have a question ,if my drivers logbooks dont match up with the maintenance record, any way we can just do it by mileage and not by date?

    No, both are needed to sync, if your drivers logs are off, then you have an issue with the driver.

    So if I'm reading this right all you have to have is all maintenance records for the last year the truck has been in service .?

    Just to clarify for you:

    The regulations also require that records of all inspection, maintenance, lubrication, repairs, and upcoming maintenance be kept for one year while the unit is in service, and for six months after the unit leaves service.The one exception is periodic (annual) inspections reports. They must contain specific information and be kept for 14 months, rather than 12.

    As Far as showing you a pic of my maint book. it would be easier for me to explain it to you what I use. I use a regular notebook for each piece of equipment. Along with the required info which i jot down on the inside front cover. I just note the date and description on service performed. I also keep a three ringer binder with any work order repair receipts/tickets.

    I don't know how others do it but, mine goes something like this. I went through all the service manuals for the engine, kw service manual and reefer service manual and wrote down all the service intervals. So I have three pages ( one page per manual ) that show that at "X" amount of miles/hours "X" needs to be preformed. Ex.

    3406e
    PM Level 1 Every 15,000 miles or 2500 gals fuel
    (cant remember the gals number, just a guess)
    Change engine oil and filter
    etc...
    etc...

    PM Level 2 Every xx,xxx miles
    ...

    (by what every the manuals said)

    Then I have another page (Service Performed) that lists;
    Date, mileage, service done, by whom, cost, next service
    Then fill it in as I go and it matches invoice and logs

    I have one more page that has each thing with pm's (engine, reefer, chassis) on it with each level pm and next miles due. Kind of a cheat sheet. Ex.

    Next Service Due
    3406e

    PM_LVL 1 xxx,xxx

    PM_LVL 2
    etc...
    SB-190
    PM_LVL 1


    As each interval comes around I cross out the current miles/hour and write down the next. This sheet just helps me keep track of the next up coming PM in an orderly fashion.

    When I got audited yesterday, he mainly looked at my page that had what I did, when and when the next one was due and it literally was just a glance at it. He didn't even scrutinize anything but I did offer up how I handle maintenance as I was getting the papers out that pertained to it and explained what each one was. I also had a stack of invoices of the work done in a folder sitting in front of him too. He was impressed, he said I had "an excellent handle on it". Again though, he really only looked at the log of what I done and when typed in some notes in his laptop and we moved on, so in the end I would have to say that that is all you really need.
