PA requires yearly state inspections as well. I assume you have an inspection place lined up there?

Get a flashlight and crawl under the truck and inspect the wheel seals for yourself. If there damp, the shop wasn’t scamming you.

PA doesn't require anything other than a normal DOT inspection that you have to show proof of for initial registration and creation of the IRP account, not for renewal. Cars get annual inspections but CMVs just have the regular annual DOT inspection that can be done anywhere in the country.

MD has a state specific safety inspection all vehicles have to go through for registration in the state. For cars, it's the only inspection that is required (MD doesn't have regular inspections for them). No clue yet if they need anything special for renewing CMV registration.

I've lived in both MD and PA for years and when I had IRP and IFTA in both states I always had to have my tractors STATE inspected for plates. I never had to get a Federal inspection because the state inspection was allowed instead. Always had my trailers registered in Maine with a Fed inspection though.

Ok. The nice part about Maryland is, it is one time inspection. Pennsylvania on the other hand has annual inspections. You can however use the Pennsylvania inspection in place on an annual FMCSA inspection.

Odd. I didn't have to do anything in 20 or 21 for PA renewal of my cab. Just filled out mv-550a, sent it and the $2417 fee to Harrisburg, and got e-mailed my cab card.

You need a pa inspection for your truck. What you described is renewing your registration. I live in PA and a state inspection is required they use to be every 6 months then switched to annual a few years back.

PA doesn't require proof of inspection when applying for plates like MD but if you don't have that state inspection sticker on your windshield they're going to break it off in you if you get caught. It's also a yearly deal instead of a one time thing like MD.
