I am having the same problem with my D12. The code is MID128 PID 94 FMI 7. Local Volvo shop tested the fuel pressure and said it was actually a little higher than spec, so they didn't think it was the pump.20psi at 600 rpm and 59psi at 1400 rpm. They changed the overflow valve and installed a fuel pump valve kit. The idle became a little smoother but I'm still getting the fault code. I had them stop work on it because they charged me $700 to get to this point and they didn't inspire me with their abilities. I got the feeling they were scratching their heads and charging me for it.

Also the fault goes active on low idle but not on high idle. Driving, no code for about 5 miles then it goes active. I don't appear to have starting issues. The engine will fire when cranked 1-3 times, it varies. Changed filters multipal times and the sensor.

Its about that time, around the seven year mark all the fuel lines coming from the fuel tanks to the engine fuel filter all need to be change, the lines develop cracks over time and start to pulling in air. I know, I had the same problem.CellNet Thanks this. -
Took the truck to Volvo in Yakima, WA today. They diagnosed it in 30 seconds. Bad accessory drive. In fact it was so bad that it was probably just a few miles from granading. I was very very lucky. Of cause it distroyed the key at the end of the fuel pump shaft, so needed a new pump. 4 hours later all new bits and new belts. They also fixed a leaky power steering union. Yakima Volvo are the BEST. I've used them many times and they have always given excellent service at an excellent price.

Have a fault code MID 128 SID 126 FMI 7
Think that code is low fuel pressure? -
Are you sure about your engine. D7E. ?? because base on experience D7C. was the last version of D7 in North America in 2003.
