I recently got a new wallet and when transferring my license, credit cards, etc to the new wallet, I must have forgot to take the medical card. And threw the old wallet away.
How do I go about replacing the medical card? Can I go to the place where I got the physical done and get a copy? Or just get a new physical? The card was only good for one year, when I went in February to get it done before starting school, I had a real bad cold/ sinus infection and I couldn't hear real well and the old, grouchy doc kept mumbling and he only gave me a 1 year card because of my hearing. But now, I could take the physical and not have any problems with that, and get a two year card.

Sooner the better. Unless you have the long form in possession, you can be cited for no medical card.

Yes you can get another card from the Doc. My original card had my last name misspelled. They gave me a replacement on the spot.

In the mean time I would get get a copy of the long form sent to me via email or fax! The carrier you work for should be able to do this immediately.

My card was getting worn out so I tried to get a new one the last time I went home. They wanted to charge me $10.00. Apparently the doctor's time is really valuable, he needs $10 for the minute or so needed to verify/sign a new card. Next time I'll charge them for the three hours I have to sit around waiting for the doctor, my time is valuable too.

Also make sure u carry one while driving a cmv,as soon as possible.Fines are really expensive.

You're a company driver, right? Contact your safety department and they'll fax you a copy to a truck stop. Ask the store clerk for the fax number or they are in the directories.

u won't have to worry about that too much longer, i forgot the date but all states have to sync together & when u get ur physical done u have a certain amount of time to give it to the dmv & they put it on ur license.
