    A few years ago I sent for a copy of my dac/report and for some one else also. I was required to send a LEGIBLE COPY of our CDL and a LEGIBLE COPY of our SOCIAL SECURITY CARD by REGISTED MAIL. This was to prove we were who we said we are. here is the info 1-800-322-9651 USIS/DAC SERVICES,INC. P.O.BOX 33181 TULSA, OK. 74153 PLEASE CALL FIRST . To make sure they have not changed their requirements or address and that you might need to include a self-addressed envelope to get your FREE YEARLY COPY OF YOUR DAC/REPORT!! Hope this helps all needing a report or who have unanswered questions of why you may not be getting that job you wanted!!:biggrin_255:

    you can only get the form you need to fill out on the website...

    you do not need to send them a stamped --self addressed envelope, they mail it to you...
    you DO NOT NEED to send it certified mail, registered mail, special delivery or over night...drop in mailbox, be done with it.....

    you CAN send it certified mail if you wish, to make sure your drivers license number and SS numbers are safe, but i never did.... regular mail....works for me.

    photo copies of your driver's license and SS card....

    you get one free copy per year.....other free copied IF you were denied a job due to a bad DAC report...

    takes (on average) 15 days from the time you mail it, to you get your free copy.

    Sorry!! I called my phone number and see it is now called hireright instead of usis/ dac services!! Thanks for the website ez money !! I will take note of the changes they have undergone!!

    scroll down to the PDF link for the form


    print it up


    fill it out


    make sure to check off the "free" report


    make photo copies of your drivers license and SS card

    put in mail

    wait about 15 days for you report


    if you dispute any information, go back to the site, down load the PDF form for disputes, fill it out, again photo copies of Dl and SS mail it.. you will then get yet another free report this time with your dispute noted....

    you do not need to send by any special mail, just put stamp and mail...

    I say BS......I just got my DAC....I filled the form out on line did not need to send them anything to get it.....got the full report took the 15 days to get here

    Whats on the report? I have been off the road for 5 years. I'm prolly gonna have to return to driving, even though I hate it.
