    Does a hiring trucking company need to see your full DOT medical exam, or just the page you carry in your wallet? Thank you

    Flat Earth Trucker Thanks this.

    Yes. They will require a copy of the longform.

    An electronic copy, email, should suffice.

    I’ve never been asked to show the full form, only the small paper you carry. I’m not sure why everyone is saying you need to show them the form.

    Kyle G., Labrador and Pickleball Thank this.

    That's because it's not required. All that's required is the short form, specifically the medical examiner's certificate. The long form is called the Medical Examination Report Form and isn't required.

    Here is the relevant regs.
    Federal Register :: Request Access

    § 391.51 General requirements for driver qualification files.
    (6)(i) The medical examiner's certificate as required by § 391.43(g) or a legible copy of the certificate.
    (6)(i) For CDL holders, beginning January 30, 2012, if the CDLIS motor vehicle record contains medical certification status information, the motor carrier employer must meet this requirement by obtaining the CDLIS motor vehicle record defined at § 384.105 of this chapter.

    There are further links to what is a certificate vs. report form, etc. in that link if one really wants to learn about it.

    Kyle G., Labrador and tscottme Thank this.

    Yeah that might be better if it just requires the short form instead of having to provide a lot of personal medical information.
