Corrective Action Plan / Safety Management Plan

Does anyone have a "Corrective Action Plan" or "Safety Management Plan" that I could use as a sample or template? FMCSA safety rating was lowered to "Conditional" for Falsifying Record of Duty Status (Logs) and I spent hours creating one and applied for a rating upgrade and it was denied even accompanied by the required 30 days of clean logs. Having to submit it to one of the load boards that requires it as well and they also rejected it. If you want to redact any info from it that's fine. I mostly need it to use as a sample of the layout they want to see. Ive searched and searched and finding nothing helpful. Thanks for any help.
