I got a letter from DVM yesterday that my current DOT medical card is expiring in a couple of months and I need to self certify. My question is, since I'm currently not driving which option on their form do I use? This is what I have to choose from.
Please HELP! I hope to start driving again in the next few months and I don't want to do something to screw up my Class A.

I'm not sure that you can self certify any more considering the new laws about medical examiners having to go thru hoops just to be able to be a qualified health examiner. If I were you, I would find the nearest examiner closest to you and get a new card. Then go to DMV and have them enter it into the system.

That's part of the form they sent me. You would think I'd you couldn't self certify, they wouldn't send you a self certify form. LOL I probably should just get a physical and be done with it. I'm sure my insurance covers it.

I plan on OTR. My current card it's good until August. DMV had my current card already.

Like any states DMV, I'm sure they are behind on things... This is from the website as of May:
Beginning May 21, 2014, drivers will be required to obtain their FMCSA medical examination from a certified medical examiner that is listed on the National Registry. Designed to improve safety by achieving high-quality medical exams that are consistent with Federal regulations and guidelines, the National Registry will help commercial motor vehicle drivers, and employees, find trained and qualified medical examiners to perform physical qualification examinations. -
If you wish to keep your CDL A, but are not currently driving then yes, you can Self Certify. This is simply letting DOT/DMV know you are not driving but retain the CDL A. It is not self certifying your medical status. I would contact the DMV and see who you need to submit the form to. I am in NC. Here the form was on-line and I simply mailed it in. I called after a month to verify they go the letter.
If you wish to start driving again, then yes you need to get a current DOT medical and submit the form again with different boxes checked. Again verify they got the letter.
If you do not self-certify then your license will be downgraded to a license for a 4-wheeler. You paid for the CDL A, keep it.
Call them to also make sure what boxes you need to check to show you wish to retain the CDL A, but expired medical. I had to call in NC as the boxes were really unclear.

Clicking on DL 8 will download a PDF that you will be required to fill out. You can fax it in. You should be Excepted, if you are not driving but wish to hold onto your CDL A.
[h=2]For Existing CDL and CDL Instruction Permit Holders[/h]If you hold a CDL or a CDL instruction permit that was issued prior to January 29, 2012 , you must provide DMV with your Commercial Driver's License (CDL) Self Certification form (DL 8) and if applicable, any medical documentation (i.e. medical examiner's certificate, also known as a DOT Medical Card, medical variance/waiver/skill performance evaluation certificate (SPE)).
Existing CDL and CDL instruction permit holders will be notified by mail of the new CDL certification and medical requirements. Included in the letter is a FAQ sheet and a Commercial Driver's License (CDL) Self Certification form (DL 8).
- If you certify to one of the excepted categories (i.e. excepted interstate or excepted intrastate), you must submit the Commercial Driver's License (CDL) Self Certification form ((DL 8)). You will not have to submit any medical documentation.

Be very careful with the "self certification" process. I discovered today that placing yourself on anything other than Teir 1 in Wisconsin is the equivalent to surrender of your CDL in Wisconsin (per the DMV). They will not renew any CDL placed in other than Teir 1 without proof of RECENT completion of a course at a CDL mill, retake all written tests, and retake the road test (on top of the road test required for the SPE Certificate), all through a third party test facility.
When I asked what I should do next, her response was that I should feel "lucky" that I'm still allowed to drive my car, and that I should enjoy my retirement. I'm 52. But, until I can work a clutch, no CDL. She thought I was making it up about big rigs being available with automatic transmissions. She's half my age, and doesn't know that already. -
I think the intentions are good but the entire "you got to do this or lose your CDL" is a bunch of crap.
By the way, most of them behind the counter barely know how to spell truck. -
Renewed my medical card but forgot to fill out self certification. Got a ticket for driving in california with a suspended cdl. Now i have to go to court and fight a misdemeanor charge.
