I have a 1999 3406e model caterpillar. 1LW19630 I had water pushing out of the blow by which I knew I either had a blown head gasket or a cracked head (Had bubbles in rad also). Removed the head and it was cracked between 2 and 3. Replaced the head made sure I had #1 dead center and pinned the motor got everything back on the truck cranked it and it has a terrible miss. Took it to my local MHC-Kenworth they hooked the computer up to it and said #5 injector was bad. No codes were thrown. They did say it showed low voltage. (wiring harness they thought would have fixed that issue) Replaced #5 injector still misses. Got a new wiring harness for the head (injectors) just in case a wire got damaged when I replaced the head. I still have a terrible miss. When you give the truck throttle it has a terrible miss. It sounds almost like a vehicle that is trying to burn water instead of fuel. I do not have any abnormal smoke from my pipes. I changed fuel filters. I put new boost pressure sensor. cam sensor.
I've done this about 3 times on this particular truck and I'm stumped. I've never had any issues once I've put a new head on the truck. There are no leaks no water in the oil absolutely nothing abnormal just a miss. Any ideas??

I would re-pin the engine again to make sure you were not 180 out. Than check and reset all of the valve clearances. Sounds to me like a tight valve or timing problem. We all want to go to the electronics first, but 80% of the time it is mechanical. First thing I was taught.

Not on a Cat engine but another tech I worked with a while back had the gear train apart on a little inline 4 Perkins. When he put it back together it was out one tooth and it gave the same symptoms you described. Not sure if you had to touch the gear train or anything but I would do as Ridindirty suggested and re-verify the timing is correct.

I gave them my engine serial number at MHC-Kenworth in order to get the head. They told me the head was interchangable with a c15 Acert head. Now they are saying they are not sure if this is correct. I know that I had to change about 3 fittings after head was installed for some of my lines to reconnect. Could they have given me the wrong head?

Forget KW, Call the local Cat house and talk with them about what heads can and can not be interchanged.

You failed to let us know about the head interchange being off of a C15.
I would for sure do as passing through says and call Cat for the right answer. Kind of scares me a little knowing it might not be the correct head. If it is the correct head, than check the timing. If not, I would be having some nasty words with your dealer. If it is the wrong head, you are #### lucky, the valves could of hit the crown of the piston, and you would be doing more than swapping a head.. -
Wow. I wouldn't think the E and Acert heads would be interchangeable but I could be wrong. I'm not near my computer at the moment or else I'd check out the numbers on SIS and see if they change up.

I think the only difference mostly would be for the iva studds. The basic head design is the same. I do know that an E model cam will not work in an acert,the injector lobes are ground differently.
Other than cylinder packs & the iva's. Most of the differences are external and electronic -
Sounds to me like your #5 injector seat is warped. Needs to be machined. Can be done while installed on engine block.
